At the foundation of hospice care is a team of skilled professionals and trained volunteers that provide physical, emotional and spiritual comfort to persons with life limiting illnesses.

Skilled Nursing
at Home Hospice Care

Registered Nurse/case manager visits at least weekly to provide care in the comfort and security of your home wherever you are – private residence, assisted living facility, or skilled nursing facility. Nurses provide guidance for patients and families confronting tough decisions and help them adapt to painful realities. Our nurses are skilled in pain management to ensure their patients remain as comfortable as possible. In addition to disease management, they also attend to the psychological, interpersonal and spiritual needs of the patient.

In Home Care

Most patients and families want to receive care in the comfort of their own home. In more than 90 percent of Hospice care cases, services are delivered in patients’ homes and/or in settings such as community-based residential facilities, assisted living facilities and nursing homes.

Temporary Inpatient Hospice Care

Hospice staff may provide services during a temporary hospital stay.

Bereavement
Care

People respond to the death of a loved one in a variety of ways. Our care service at Legacy Now Hospice extends beyond the treatment of physical symptoms, to the emotional and social needs of patients and their families. Our professional team of counselors and social workers is here to support you through your loss.

Emotional and
Spiritual Support

Grief can sometimes make us question our beliefs, but it can also be a time of reflection. Because hospice care focuses on the whole individual, it also includes therapy related to emotional issues surrounding a terminal diagnosis and end-of-life concerns. Our highly experienced staff can provide care for the spiritual needs brought on by a life-limiting illness. We can also help you to find solace in your faith, whatever your beliefs. Chaplain provides spiritual support to patients of all religious backgrounds.

Hospice aid services

Patients with a serious illness often have trouble with activities such as personal grooming and household tasks. Certified nurse aide visits multiple times a week as needed to assist with personal care.
They interact with the patient and help with activities of daily living during visits. The home health aide may educate family members and other caregivers on patient care so that the families will feel comfortable providing day to day care to the patient between home health aide visits.

Social Worker Support

We work with a team of qualified and experienced social workers who help support everyone from children to adults in facing the emotional and psychological consequences of someone close to them dying.

Respite care

When family members and loved ones are caring for someone with a life-limiting illness, they can sometimes feel they have no time for themselves. Respite programs provide planned short-term and time-limited breaks for families and caregivers in order to support and maintain the primary care giving relationship. Respite also provides a positive experience for the person receiving care. If your family caregiver needs a rest or some time away to simply recharge, Legacy Now Hospice team can provide short-term relief.

Care
coordination

We provide coordination of care for patients being looked after at home. These services may include appointments to see a consultant, regular visits by registered nurses, hospice aides, social workers, a Pastor/Chaplain, therapists, or other support services provided by hospice.

Personalized care

Our goal is to help you experience the best possible quality of life. We’ll work with you and your family to build a care plan specifically for your needs. We also provide the necessary medications, equipment and supplies to support your hospice diagnosis.
